This study investigates the comparative effectiveness of single-inhaler triple therapy (SITT) versus multiple-inhaler triple therapy (MITT) in treating patients with Chronic Obstructive Pulmonary Disease (COPD) in Spain. It analyzed electronic health records from the Spanish National Healthcare System BIG-PAC database from June 2018 to December 2019 to determine differences in treatment persistence, exacerbation rates, and health care resource utilization among COPD patients aged 40 years or older. <br /><br />With 4,625 patients (1,011 initiated on SITT and 3,614 on MITT), the study found that SITT users demonstrated superior treatment persistence at both 6 and 12 months, compared to MITT users. Specifically, SITT patients had a 37% improvement in persistence, a 32% lower risk of exacerbations, and a 33% reduced all-cause mortality compared to MITT users. <br /><br />Furthermore, SITT was associated with fewer exacerbations (both moderate and severe types), longer time until the first exacerbation, fewer healthcare visits (primary, specialized, emergency), reduced hospitalizations, and shorter hospital stays. These factors contributed to a cost-saving effect, with SITT associated with an adjusted mean annual cost saving of €403 compared to MITT. <br /><br />Persistence, in general, was linked to improved exacerbation rates and substantial healthcare cost savings in both therapy groups. The study highlights the clinical benefits and economic advantages of using SITT over MITT, emphasizing improved treatment adherence and reduced healthcare resource utilization. <br /><br />This retrospective cohort study supports the preference for single-inhaler regimens in COPD management to enhance patient adherence, leading to better health outcomes and efficiency in healthcare resource allocation. The findings suggest adopting a single-inhaler approach could mitigate COPD-related morbidities and overall treatment costs.
