Pdf Summary
The narrative review by Aaron Yarlas et al. presents a comprehensive evaluation of patient-reported outcome measures (PROMs) used to assess health-related quality of life (HRQoL) in individuals with pulmonary hypertension (PH). Given the rising effectiveness of PH treatments, the review emphasizes the increased importance of including patient perspectives on their well-being as a measure of treatment benefits. The authors assess several PH-specific PROMs, including the Cambridge Pulmonary Hypertension Outcome Review (CAMPHOR), emPHasis-10, Living with Pulmonary Hypertension Questionnaire (LPHQ), and Pulmonary Arterial Hypertension—Symptoms and Impact (PAH-SYMPACT).

The paper highlights that PH, which can lead to several subtypes such as pulmonary arterial hypertension (PAH) and chronic thromboembolic pulmonary hypertension (CTEPH), significantly impairs patients' physical, social, and emotional functions. Therefore, PROMs are crucial in capturing patients' experiences, supplementing objective measures like hemodynamics and survival rates, to understand better the impact of treatments from the patient's perspective.

The review found that, while all four PROMs were developed according to best practices by involving patient input, each measure has unique strengths and limitations. CAMPHOR captures comprehensive disease impacts including ADLs and emotional functioning but is lengthy and may require frequent assessments. EmPHasis-10, although brief and efficient, misses several critical symptoms and activities due to its limited coverage. LPHQ effectively captures numerous disease impacts with fewer questions but needs more reliability and validity data. The PAH-SYMPACT is unique in capturing cardiovascular symptoms but lacks data on treatment sensitivity.

The authors conclude that no single PROM is universally superior, suggesting that careful consideration of study context and objectives is required in selecting an appropriate PROM. They advocate for the continuous evaluation of these measures as more data becomes available, emphasizing that selected instruments should accommodate various patient populations and disease contexts within the evolving landscape of PH treatment.
