The corrected recommendation states that UFH should be administered at a rate of 5 u/ml at 1 ml/hr, maintaining a Grade 1A evidence level. This error was identified in the guidelines published in "Chest 2012;141(2)(Suppl):e737S-e801S" on pages e739S, e763S, and reflected in the Executive Summary on page 42S. These changes have been rectified in the online version of the document to ensure accuracy in clinical practice.
