Pdf Summary
The diagnosis of sarcoidosis, a complex granulomatous disease, remains largely arbitrary and involves circular reasoning despite the existence of seemingly established criteria. Traditionally, the diagnosis hinges on clinical-radiological findings supported by histologic evidence of non-caseating granulomas, but these are subjectively defined by the clinician's opinion. The American Thoracic Society, European Respiratory Society, and World Association of Sarcoidosis emphasize excluding other granulomatous diseases, yet this is inherently challenging because complete exclusion of alternative diagnoses is never entirely achievable. The requirement for a second organ presentation also fuels controversy, although some clinical presentations, like Löfgren’s syndrome, suggest a single organ can suffice for diagnosis.

Judson suggests learning from the diagnostic criteria setup for systemic lupus erythematosus, where impartial consensus criteria help objectively assess the diagnosis's likelihood. Similarly, Bickett et al. attempted to establish diagnostic criteria for sarcoidosis by designing a Sarcoidosis Diagnostic Score (SDS) through an assessment of 553 biopsy-confirmed cases and 427 controls, leading to a new scoring system improving diagnostic specificity.

Their study, while pioneering, is limited by being conducted within a single institution, which may not reflect the sarcoidosis phenotypic variability across different populations and might lack representation of diseases often confused with sarcoidosis, potentially inflating specificity results. These limitations can be mitigated with further studies on a broader international scale. Nonetheless, their work represents a crucial step toward a more standardized diagnostic framework, moving sarcoidosis diagnostics from arbitrary decision-making to a more systematic approach.
