Pdf Summary
The Fleischner Society's multinational consensus statement emphasizes the crucial role of chest imaging in managing patients during the COVID-19 pandemic. Given the heterogeneous spread of the virus, the panel of global experts assessed the utility of imaging, particularly chest radiography and CT, across different scenarios characterized by varying severity, risk factors, and resource constraints. The panel, comprising radiologists and pulmonologists from 10 countries, formulated recommendations based on real experiences from varying health care settings. They used three primary scenarios to determine when imaging is most effective: 1. Mild Features of COVID-19: Imaging is generally not recommended unless patients with mild symptoms are at risk for disease progression. This approach helps prioritize resources and minimize unnecessary exposures. 2. Moderate-to-Severe Features of COVID-19: In cases with moderate-to-severe symptoms, imaging is advised, regardless of COVID-19 test results, to establish a baseline and identify potential complications like superimposed bacterial pneumonia or heart failure, particularly if clinical conditions worsen. 3. Resource-Constrained Environments: Here, chest imaging supports triage when testing resources are scarce. Imaging can provide a swift diagnosis to manage patient flow and resources effectively, likely offering presumptive COVID-19 diagnosis in high pretest probability scenarios even when tests are negative. Furthermore, routine imaging in asymptomatic patients is discouraged unless they are at risk of progression. Imaging after recovery could be necessary for patients with lingering symptoms to differentiate between COVID-19 sequelae and other conditions. The panel also highlighted that rapid COVID-19 testing could significantly aid management decisions but noted the limitations posed by resource availability and operational logistics of administering and utilizing imaging effectively in the pandemic response. These recommendations underscore a flexible approach, integrating imaging with the understanding of local resource allocation, patient presentation, and health care capabilities, to optimize COVID-19 management efforts globally.
